Services We Offer

Home-Delivered Meals

● Meals are delivered Monday through Friday.

● Our kitchen begins cooking at 7:00AM, and meals are on the road for delivery by 9:30AM.

● We offer hot, cold, and/or frozen meals for delivery.

● We deliver directly to consumers located in Lackawanna, Luzerne, and Wyoming Counties.

● Meals are delivered by either driving staff or community volunteers.

● All meals are heart healthy and diabetic friendly – we do not add any salt or sugar.

● All meals are made fresh in our kitchen, with seasonal, locally-sourced ingredients.

● All meals come with a side of fresh fruit or a fruit cup, a half pint of milk, and a serving of whole grains.

● We accept referrals from:

      The Lackawanna County Area Agency on Aging

      State Medicaid/ Waiver managed care organizations (AmeriHealth Caritas, PA Health &

      Wellness/Centene, and UPMC)

      The Wright Center

      LIFE Geisinger

● We also offer a private pay program, for $6 per meal to consumers in Lackawanna, Luzerne, and Wyoming Counties. Anyone is eligible.

Community Health Initiatives

Fast Track Home from the Hospital Program

  • Provides up to 20 meals free of charge to adults over the age of 18 who live in
  • Lackawanna County and were recently discharged from a hospital, nursing home, or rehabilitation facility.
  • Referrals are usually made by social workers helping the patient at the time of discharge from a facility or a care taker.
  • The goal of the program is to provide necessary nutrition to aid in the healing process, while decreasing the community’s costly readmission rate.
  • 100% funded as a community health program by the United Way of Lackawanna, Wayne, and Pike Counties.


Community Health Outreach and Meal Program (CHOMP)

  • Integrates staff, volunteers, and local health care students into our community service delivery.
  • CHOMP provides the extra services needed to maintain clients' independence in their
  • communities with safe, clean environments.
  • Graduate & Medical students complete monthly wellness assessments on participants.
  • The students gain Interprofessional service learning experience.
  • Community health workers visit the clients once a week and complete light housekeeping and socialization.
  • Less clutter lowers fall-risk.
  • Clean environments benefit clients’ health and emotional state.
  • Weekly visits have a positive impact on socialization.
  • Keeping a clean home/apartment can prevent evictions.
  • Program participation can be referred by the Lackawanna County Area Agency on Aging or a Social Worker.
